GUIDELINES FOR FORMS AND LETTERS


1.  Application forms should always be returned to the Formation Director who will review the forms and keep them on file.

2.  Application for the initial orientation sessions -- to be given out at the first information session.

3.  Application to the Inquiry Phase to be given at the first Inquiry session.

4.  Interviews should be conducted at some point during the Inquiry phase by the Formation Director and the Spiritual Assistant and presented to the Fraternity Council.  Applications and interviews should be carefully reviewed.  At this point,  letters of recommendation -- one from the pastor, another from a professed member of the fraternity -- should be required of the candidates. The council makes the decision  to accept or not the candidate to the Formation phase.  Recommendation to further inquiry sessions can be made, if need arises.  If decision not to accept is made, Formation Director and Spiritual Assistant should lead the inquirer gently toward that discernment.

5.  Application to the Formation sessions should be given out at the first Formation session.

6.  At the end of the Formation phase, the candidate should make a decision whether to become a fully professed member, or make a temporary profession.  Send acceptance letter to the candidate and make plans for profession ritual.
