I N S T R U C T I O N S

1. On the front of the index card, at the top, print your name, last name first.
2. Underneath, write your major and the number of credits you have earned (at this school or elsewhere).
3. If you have a job that uses computers, put it at the bottom.
4. In the lower left corner, add today's date and the number of this course.
5. On the back, state your reasons for taking this course.
6. Optional: You may list a phone number to call for class cancellations, etc.
   Also, if you wish to be contacted (after the semester is over) about job or business opportunities, write "OK to contact".
